Title: An analysis of risk factors affecting the health and mortality of neonates with a very low birth weight born in the Province of Opole in the years 1998-2003

Abstract:

The aim of the study is to evaluate the population of newly born babies with a birth weight of ≤ 1500g born in 1998-2003 in the Opole region as well as considering the influence of the risk of death and the development of the child after 1 year of age. It is a retrospective cohort research concerning 402 preterm infants in two groups (I 1998-2000, II 2001-2003. Survival and morbidity rate were compared in group I and II. The development analysis concerns 112 newborns whose full anthropometric measurements and neurological diagnosis (Vojta’s method). The frequency of cerebral palsy, sight and hearing impairmet were also assessed. Death risk factors and disturbed development factors were confronted. Overall survival rates for epochs 1 and 2 were increased (ELBW). Mothers of these newborns received comparably corticosteroids (28,1%). Group II newborns more often suffered from infections, PVL, PDA and 3rd level ROP. At the same time more intensive care procedures were carried out during the stay in hospital. The most influential death risk factors for preterm infants in Opole region were birth weight, gestational age, Apgar scale result at 1st, 3rd, 5th minute of life, place of birth and antenatal steroid therapy. Delay in somatic development was observed among newborns from group II in regards to body weight and body length. There were observed no differences in the psychomotor de ; velopment in number of infants with movement disturbances (CP, eyesight defect, hearing defect). Factors which could have influenced infants incorrect development in group II were birth weight, transport by ambulance N, an infection, BPD, 3td level ROP, PVL, IVH. Conclusions 1. Better neonatal care in Opole region increased the level of the survival chance of the least mature newborns. 2. Premature infants born with ELBW are significantly endangered with psychomotor development disturbances. 3. One of the reasons for such a situation is low level antenatal steroids therapy applied in the case of a risk of a premature birth. 4. Eyesight in comparison to hearing is more often endangered with disturbances connected to immaturity. 5. Implication of periodical analysis of deaths in different regions of the country can be helpful to improve prenatal care in a specific region.
