Title: Evaluation of vascular endothelial function and leukotriene synthesis in patients with atherosclerotic occlusive disease of the lower limb arteries following intravascular treatment

Abstract:

The contemporary concept of atherosclerosis emphasizes the role of inflammation in its etiopathogenesis and the involvement of potent proinflammatory mediators, such as eicosanoids, leukotrienes (LTs) and thromboxanes (TX), is taken into account. The associations between LT concentrations and endothelial function changes have not been prospectively investigated in peripheral arterial diseases thus far. In Stage I, a systematic review of the published literature on the potential role of LTs in the pathophysiology of cardiovascular diseases was carried out. In Stage II, the association between urinary levels of LTE4 and LTB4 and parameters reflecting non-invasive measurements of endothelial function, as well as potential correlations with clinical characteristics and the achievement of a composite endpoint in the form of restenosis in the lower limb arteries or death during one-year follow-up after endovascular treatment were assessed. In Stage III, the relationships between vascular inflammation represented by the concentrations of leukotrienes E4 (LTE4), 5-hydroxyeicosatetraenoic acid (5HETE), and thromboxane B2 (TXB2), and the dynamics of changes in quality of life in patients undergoing intravascular treatment for peripheral arterial disease (PAD) were examined. The results indicate a significant impact of leukotrienes E4 (LTE4) and B4 (LTB4) on vascular endothelium and i ; ts functioning in patients with PAD. It has been demonstrated that the values and magnitude of changes in LTE4 and TXB2 levels were positively correlated with the quality of life during the 12-month observation period.
