Title: Etiopathogenesis of multiple myeloma

Abstract:

Introduction: Multiple myeloma (MM) is a plasma cell malignancy, characterized by the production of a monoclonal protein accompanied by end-organ damage. Confirmed risk factors include: older age, male sex, African-American race, and family history of multiple myeloma in the first degree relative. The aim of the study was to search for other potential prognostic factors of the development of MM. Material and methods: A case-control study was conducted in 20 hematology centers all over Poland and in 5 outpatient clinics in Kraków and its vicinity. The questionnaire on exposure to potential risk factors (including sociodemographic data, lifestyle, environmental factors, comorbidities, previous treatment, and family history) was completed by both the patient and the physician. Results: 274 patients with newly diagnosed multiple myeloma and 208 patients in the control group were enrolled in the study. Negative prognostic factors are older age (OR=1,048) and male sex (OR=2,152), while positive factors include: living in a city over 50,000 inhabitants (OR=0,228), as well as regular physical activity for a period of at least 6 months (OR=0,512). Conclusions: The pathogenesis of multiple myeloma is complex, with the impact of environmental factors, sociodemographic conditions, genetic background and medical history. Lifestyle, with physical activity and professional activity, seems ; to play a key role.
