Title: Evaluation of molecular mechanisms of action of negative pressure wound therapy in the treatment of neuropathic ulcers in diabetic foot syndrome

Abstract:

Diabetic foot syndrome is frequently occurring together with ulceration (DFU). Negative pressure wound therapy (NPWT) is adjuvant method in DFU treatment. NPWT molecular mechanism of action is not fully understood. The objective of this doctoral thesis was evaluation of selected aspects of clinical use and molecular mechanisms of action of NPWT in type 2 diabetes mellitus (T2DM) patients with DFU. The assignment to NWPT was non-randomized and based on wound characteristics, clinical guidelines and local clinical practice. Groups were similar in clinical characteristics. The NPWT group had greater wound area. The study proved that NPWT is not more effective than standard therapy, however, due the greater DFU area, one may hypothesizes that NPWT is more effective. Interestingly, the rate of ulcer recurrence was almost the same in both groups, in spite of a large difference in the magnitude of their initial area. The effect of NPWT in DFS may be mediated through differential gene expression of proteins involved in inflammatory process and through alterations of DNA methylation resulting mainly in inhibition of the complement system activation. These findings may be of value in guiding the development of new therapies for DFUs. However, it requires further confirmation in subsequent studies.
