Obesity is associated with a reduction of natriuretic peptides (NPs) concentration in patients with heart failure (HF). NPs levels may also be affected by components of the metabolic syndrome (MS) and single nucleotide polymorphisms (SNPs) in genes related to NPs.Purpose: Evaluation of the relation of NT-proBNP concentration with selected SNPs in patients with chronic HF and MS.Study group and methods: 88 patients with chronic HF with LVEF≤45% and MS, were divided into two groups: with obesity defined as BMI≥27kg/m2 and without obesity defined as BMI <27kg/m2. The NT-proBNP blood test, transthoracic echocardiography, body composition analysis with bioimpedance and genetic analysis of SNPs: (-381) T/C of NPPB gene, (-15129) 3C/4C of NPR1 gene and (-55) C/A of NPR3 gene were performed. The clinical observation lasted 24 months.Results: NT-proBNP concentration was negatively correlated with body weight in whole population, and with trunk lean mass in the non-obese group. The groups did not differ in respect to selected SNPs. In patients without obesity, the genotype (-381) T/C of the NPPB gene was associated with higher NT-proBNP concentration. Genotype (-15129) 3C/3C of NPR1 gene was associated with higher blood pressure , and genotype (-55) A/A of NPR3 gene was related to impaired fasting glucose and impaired glucose tolerance. Genotypes (-15129) 4C/3C and 3C/3C of NPR1 gene wer ; e associated with mixed hyperlipidaemia. There was no significant relations of SNPs or obesity to prognosis. The waist-hip ratio was an important predictor of re-hospitalization in patients with HF and MS.Conclusions: Regulation of NPs concentration is multivariate. Influence of MS components may be more important than genetic modulation. MS components may be associated with SNPs in genes related to NPs.
