Title: Anatomical and echocardiographic assessment of the mitral valve and the mitral subvalvular apparatus - implications for percutaneous and surgical methods of the valve diseases treatment

Abstract:

The mitral valve complex consisted of annulus, leaflets, commissures, tendinous cords, papillary muscles, the left atrial and ventricular walls, and is one of the most complicated mechanical arrangement in the human body. The rapid development of surgical and less-invasive mitral valve repair procedures has greatly increased the interest in mitral valve anatomy.This study describes anatomical variants and morphometry of the mitral valve leaflets as well as the main distances between the mitral annulus and adjacent structures in the 200 autopsied human hearts. In all cases, the mitral valve is built by two main leaflets with possible variants in scallops (29.5%). The variations are largely associated with posterior leaflet and are mostly related to the presence of accessory scallop. Anatomically, the anterior mitral leaflet is not divided into scallops, but could have accessory scallop (2.5%). Papillary muscle and tendinous cords morphologies were characterized based on 100 hearts. There is a high variability in the papillary muscles and tendinous cords in the mitral valve complex. Proper nomenclature, simple classification, and the most common variants for papillary muscle groups and tendinous cords were presented. Last part of the study evaluated the effect of MitraClip implantation on severe functional mitral regurgitation in comparison to conservative therapy. We have included consecutive 33 patients assigned to two groups. Patients treated with the MitraClip achieved significant reductions of symptoms, mitral regurgitation grade and improvement in quality of life in comparison to conservative treatment.
