Infective endocarditis (IE) is an infection caused by one of many pathogenic microorganisms (bacteria and/or fungi), which primarily occupies the heart valves, leading to their destruction and subsequent development of regurgitation. This disease can also develop on other parts of the myocardium or blood vessels, as well as on implanted parts like heart valves, pacemaker electrodes or vascular prostheses. Due to the narrow therapeutic range of vancomycin, this antibiotic should be monitored by the concentration of the drug in the blood of patients.The aim of this study was retrospective analysis of pharmacokinetic parameters and assessment of the usefulness of PK/PD criteria in predicting the efficacy and safety of empirical and targeted vancomycin therapy in patients with documented IE, after aortic and/or mitral valve replacement surgery. The analysis was based on data from the pharmaceutical interview, supplemented with information from the medical history records of patients hospitalized in 2010-2018 at the Krakow Specialist Hospital John Paul II. Based on clinical data recorded in the medical history charts, pharmacokinetic analysis of vancomycin was performed in selected patients, based on traditional monitoring of vancomycin (Cmin and Cmax) and on new ESC guidelines including measurement of Cmin only. Due to the lack of information on the participation of a pharmacist in ; the Endocarditis Team in the currently applicable guidelines for the treatment of IE, the second objective was to prospectively analyze the clinical pharmacist's procedures in the care of the IE patient and indicate his tasks and scope, which may contribute to a reduction of mortality among IE patients, as well as to improve the effectiveness and safety of pharmacotherapy in patients with IE.
