Title: The diagnostic value of MR diffusion techniquesin the differentiation of brain tumors
Abstract:
The aim of the dissertation was to evaluate whether technology diffusion MRI can be used in the diagnosis and differentiation of brain tumors. A retrospective analysis of 155 studies which underwent diffusion MR was performed on adult (104) and paediatric (49) patients with histologically confirmed tumors of the brain. Among all analysed, 59 cases were diagnosed with primary benign tumors, 61 with primary malignant tumors and 35 cases with metastases. The control group consisted of 100 adult patients and 40 pediatric patients without histologically MRI confirmed tumors. We analyzed the DWI signal values and ADC ratios with and without contrast enhancement in the areas of tumors and peritumoral brain edemas. A reference point of interest was agreed at a signal derived from normal nervous tissue located symmetrically with respect to the tumor and homologous structure brain/cerebellum of the control group. The value of the signal from the DWI of the tumor and peritumoral brain edemas increases in adults and decreases in children with the degree of malignancy of the brain. The tumor’s ADC value decreases whereas ADC of peritumoral brain edemas decreases with the degree of malignancy of brain tumor. Benign tumors show a lower value of the DWI signals and higher ADC rates compared to the normal nervous tissue. Malignant tumors show higher DWI signal values and higher ADC coefficient ; values with respect to the normal nervous tissue. The introduction of DW-MRI sequence for routine diagnosis of patients with pathological changes in the CNS provides important information that can help in the differentiation of focal lesions of the brain.
