Title: Aortic valve implantation through upper ministernotomy-clinical evaluation

Abstract:

The aim of the dissertation was to compare the intra and postoperative data from patients undergoing aortic valve replacement, either through ministernotomy (Mini- AVR) or conventional sternotomy (AVR).Medical data of 150 patients who underwent aortic valve replacement between January 2011 and December 2012 were analyzed in a retrospective manner. Two groups were evaluated: (I) Mini-AVR group consisting of 74 patients and (II) AVR group consisting of 76 patients. Compared groups were homogeneous in terms of demographic factors, preoperative laboratory test results and echocardiography parameters.The analysis of the obtained data showed statistically significant differences between both groups of patients in intra and postoperative period. During the operations aortic cross-clamp time and cardiopulmonary time were longer in Mini-AVR group. There were also observed shorter mechanical ventilation time, hospital stay and lower post operative drainage in Mini-AVR group. The biological prosthesis were more frequently implanted in Mini-AVR group.Furthermore patients from Mini-AVR group reported less postoperative pain.The type of surgical approach did not affect the frequency of postoperative complications such as mortality, stroke, atrial fibrillation, renal failure, wound infection, sternal instability or the need for rethoracotomyObtained results lead to conclusion that surgical approach via ministernotomy for aortic valve replacement is a safe method and does not increase morbidity and mortality.
