Neurosyphilis may occur at every stage of syphilis. When untreated, may progress in a hidden manner for many years, leading to late neurological and psychiatric complications, which may result in permanent disability. The aim of the current study was to assess the immune response in early syphilis by measuring the levels of cytokines typical for innate immune response (TNF-α, IL-1β), Th1, Th2, Th17 and antinflammatory responses (IFN-γ, IL-4, IL-17A, IL-10, TGF-β, respectively) in cerebrospinal fluid (CSF) and serum of syphilitic patients. Significantly higher levels of CSF IFN-α, IL-17A, TNF-α, IL-10 and TGF-β were found in patients with neurosyphilis when compared to those with syphilis but not neurosyphilis. Moreover, in patients with neurosyphilis the relative advantage of antinflammatory response in CSF was found. It may cause adequate conditions for the survival of bacteria and chronic infection.
