Title: The evaluation of the FreeLite assay - quantification of theimmunoglobulin free light chains for the detection andmonitoring monoclonal gammapathies in comparision withelectrophoretical methods in patients with multiplemyeloma


Monoclonal gammapathies are a group of disorders characterized by clonal expansion of B cells. The recognition of monoclonal protein may require multiple approaches and the use of many different diagnostic tests. There are various screening algorithm panels for the detection of monoclonal gammapathies but it is not so far, the standard test procedure. In recent years new automated immunoassay measuring serum free light chains (FreeLite®) have been developed. The assay is useful for the diagnosis and follow-up of amyloidosis, light chain disease and for the prognosis of MGUS and smouldering myeloma. In the current study the results concerning methodological difficulties with the FLC assay were presented. It was found that the FLCs concentration evaluated by FreeLite® assay might be overestimated and that there are some inconsistencies in relation to the results of another diagnostic test. Results of this investigations shows the limits of FreeLite® assay. The cause of these restrictions may be sample dilution anomalies (non-linearity) or failure of antigen excess detection and immunological free light chains properties. In presented study the correlation of FLCs serum levels and serum level of beta-2-microglobulin of patients with monoclonal gammopathy was evaluated. A significant correlation of serum levels of the lambda dominant chain and beta-2-microglobulin (p=0,0000295; r=0,703) was found. Laboratory professionals and clinicians should be alert to analytical difficulties with the FLC assay. The limitations of FreeLite® assay canmislead and complicate clinical interpretation of a FLC result. Therefore it is advisable to perform FLC analysis of diagnostic and follow-up sample sunder the same analytical conditions, using more than one dilution on the same instrument. The assay should be performed in good dialogue between biologist and clinician, and in combination with other diagnostic tests.
