Title: Assess the efficiency of the stable osteosynthesis as treatment of zygomatico-maxillo-orbital complex fractures

Abstract:

The aim of the study is to assess the efficiency of the stable osteosynthesis as treatment of zygomatico-maxillo-orbital complex fractures. The following research questions were posed: 1. Does the final result of the treatment of zygomatico-orbital complex fractures depend on: a) the period of time lapsed from the trauma to the surgical treatment? b) the form of the fracture and the type of coexisting injuries? 2. Is there a statistic dependence between the type of preoperative diplopia and the ultimate results of the surgical treatment? Between the years 1998-2002 (5 years), 3802 patients were hospitalized in the Clinic ward including 1330 (35.0%) patients with head and neck injuries. 496 patients were hospitalized due to the isolated zygomatico-maxillo-orbital complex fractures or the zygomatico-maxillo-orbital complex fractures coexisting with other injuries. The data of 170 patients who underwent surgical reduction and stable micro- or miniplate osteosynthesis of zygomatico-maxillo- orbital complex fractures were analyzed in-depth. The optimal period of time between the trauma and the surgical treatment in the zygomatico-maxillo-orbital complex fractures amounts to 7 to 14 days. The performance of surgical treatment after this time worsens the results of aesthetic and functional recovery. The percentage of recoveries reached statistical significance in the cases of ; the patients with type I and II of posttraumatic diplopia. The worst treatment results were observed in the cases of the patients with type V of diplopia. The analysis of clinic material proves the efficacy of the diagnostic and therapeutic method emplyed with the patients with the zygomatico-maxillo-orbital complex fractures by the Department of Cranio-Maxillo-Facial, Oncological and Reconstructive Surgery in Cracow.
