Title: Effectivenes of plevic organ prolapse surgery when paravaginal defect repair is performed

Abstract:

Objective: The aim of the study was to assess the prevalence of paravaginal defect (pd) among patients with pelvic organ prolapse (pop) qualified to operation and to compare the results of pop surgery when pd repair was performed. Material: One hundred and nine patients with symptomatic pop who were qualified to operation referred were enrolled to prospecture study. Among them two groups were identified: group I (N = 49) with pop and pd and group II (N = 60) with pop but without pd. The preoperative assessment of all patients included a thorough history designed to elicit specific details of the general medical status and their urinary complaints. Preoperative and postoperative evaluations were performed with the use of Pelvic Organ Prolapse Quantification System. Additionally, all patients were observed to have paravaginal defect before and after operation. Results: Percentage of patients who had grade 0 or 1 in group I and II was as follow: 6 days after operation (95,9% vs 96,7% p = 0,8362), 3 months after operation (70,8% vs 82,5% p = O, 1577) and 6 months after operation (63,6% vs 80,0% p = 0,0768). The reduction of cases of stress urinary incontinence was 65,3% in group I and 41,6% in group II respectively. Pd was found in 44,95% of patients qualified to pop operations. The cure rate of paravaginal defect repair during 6 month observation was 90,9% and pd appeared de novo ; postoperatively in 12% of patients who had not it before operation. During 6 month follow up there was no statistical difference in prevalence of grade II or higher in group with corrected pd and group without paravaginal defect before operation. Conclusion: Paravaginal defect repair should be included to complex operative treatment of pel vie organ prolapse.
