Title: The usefulness of cytokines blood serum level in patients with acute pancreatitis

Abstract:

The subject of the study were 103 patients with acute pancreatitis (AP). There were 57 patients with biliary and 46 patients with post­ alcoholic. Patients were divided into two groups; edematous pancreatitis (84 patients) and necrotizing (19 patients). Samples of blood from peripheral vein were obtained at 1, 3, 5 and 7th day of hospital stay for assessment of interleukins. Interleukins were assessed by ELISA method using Biosource Europe S.A. kit. The aims of the study were: to evaluate the changes of interleukin levels in the different etiological forms of pancreatitis - biliary and post-alcoholic, to assess which interleukin has the h ighest c orrelation w ith e tiology and severity of the disease, to follow the levels of different interleukins after papillotomy in biliary pancreatitis and to assess whether interleukin-8 may be a marker of necrosis. Important differences in the level of interleukin-6 were observed form the first day of hospitalization and during following days between edematous and necrotizing pancreatitis. Similar differences were observed in levels of interleukin-8 but from 3rd day on and interleukin-10 in the 1st and 5th day. Important differences between biliary and post-alcoholic pancratitis were noticed in the levels of interleukin-2 throughout the study and interleukin-12 in the 1st and 3rd day. Early papillotomy caused significant decrease mean leve ; ls of interleukin 1, 2 and 10. Statistical significance of the difference in the interleukin-8 level between edematous and necrotizing form increased form the 3rd day, and on which may point to the value of it as a marker necrosis. Levels of interleukin-6 and interleukin-10 correlated well with severity of the disease measured by APACHE II scale. Similar observations were made in case of Trapnell’s classification.
