Title: Effect of early abciximab in ST-elevation myocardial inrafction patients treated with mechanical reperfusion on outcome in high risk subgroup

Abstract:

There were 1650 patients with STEMI gathered in EUROTRANSFER registry database who were treansferred do invasive cardiology center in 15hospitals in 7 european countries in 2005-2007. Of these, 1086 received abciximab during their course of treatment. Patients were then divided into two groups: early (EA) and late (LA) abciximab administration. Both groups did not differ in terms of demographic, clinical and baseline angiographic characteristics. Patency of infarct-related artery defined as TIMI 3 or TIMI 2+3 was significantly better in EA group (TIMI 3 - EA vs LA: 17.7% vs 8.9%, p<0.0001). In-hospital and 30-day death and death, reMI at 30 days and composite 30 -day endpoint of death, reMI, urgent revascularisation were more frequently observed in late abciximab group. There were 42 deaths (5.8%) in early abciximab group in comparison to 37 deaths (10.3%) in late abciximab group in 12-month follow-up (p=0.007). Early abciximab administration in patients with STEMI treated with primary PCI results in siginificantly lower in-hospital and long-term observation (30-day, 1-year) mortality in comparison to late abciximab administration. This effect on mortality reduction in early abciximab is especially seen in high risk patients (according to TIMI scale), over 70 years of age, women, patients with diabetes mellitus and early comers with time from chest pain onset less than 3 hours.
