Title: Detection of minimal residual disease in children with acute lymphoblastic leukemia treated according to the ALL IC-BFM-2002 Protocol

Abstract:

From May 2006 to January 2008 minimal residual disease (MRD) was monitored using four-color flow cytometry in 50 children with acute lymphoblastic leukemia (ALL) treated according to the ALL IC-BFM-2002 Protocol in Oncology and Hematology Department, Polish-American Institute of Pediatrics, Jagiellonian University Medical College. The aim of the study was to assess dynamics of MRD level changes in peripheral blood (PB) and bone marrow (BM) and to define statistically significant measurement time points and cut-off level of MRD, which are the superior estimate of the hazard of treatment failure. The study showed that statistically significant cut-off level of MRD, that best estimated the hazard of treatment failure depended on a point of measurements. Children with MRD level on day 15 ≥0.1% and ≥ 0.0085% on day 78 in BM were at highest risk of treatment failure. Additionally, statistically significant correlation was shown between high MRD level and classical risk groups, presence of t(9;22) and response to treatment. Further MRD studies could lead to better recognition of leukemia biology, as well as to further individualization of therapy and improvement of cure rates in ALL.
