Title: Plasma concentrations of insulin and insulinlike factor 1 in burned children


The purpose of the study was to determine disturbances of the plasma concentration of insulin (IN), C-peptide (CPP), insulinlike factor 1 (IGF-1) and it’s binding protein 3 (IGFBP-3) in the group of children after severe burns, treated with the use of aggressive method and, if these changes depend on: burn area, local infection or sepsis. Blood samples were collected from 30 severely burned children on the 1-st, 2-nd, 4-th, 9-th and 21-st day after injury and from 27 healthy children. CPP plasma concentrations were significantly elevated above the upper limit of the normal range. IGF-1 and IGFBP-3 concentrations were significantly decreased, but its levels were within normal range. IN concentrations were significantly increased, but still within reference ranges. We founded significant correlations between the burn area and the intensity of changes of the plasma levels of IN, CPP, IGF-1 and IGFBP-3. Another correlation was revealed between the variations of IN and CPP plasma concentrations and the presence of local or systemic infection. Observed alterations of tested parameters are slight. The effects result from the aggressive method of treatment of burn disease and that such treatment shortens the catabolic phase of stress response.
