Title: Left ventricle diastolic function assessment in acute myocardial infarction patients treated with primary angioplasty

Abstract:

The aim of the study was to evaluate left ventricle diastolic abnormalities in acute myocardial infarction patients treated with primary PCI, coexistence of the left ventricle (LV) diastolic and systolic abnormalities in acute myocardial infarction patients treated with primary PCI and changes of the left ventricle filling pattern 3 months after primary PCI. This prospective study consisted of 99 patients aged between 30 and 70 years with acute ST-elevation myocardial infarction treated with primary angioplasty. It was established that primary PCI effectiveness had influence on diastolic dysfunction severity within 24 hours after PCI (E1). After analysis of the average values of TnI max, CPK max and CK-MB max it was proved that levels of cardiac biomarkers were significantly higher in restrictive filling pattern patients. Changes of the diastolic function within 3 months after primary PCI were observed. Conclusions: nearly 70% of patients with first STEMI infarction present LV diastolic dysfunction within first day after PCI; myocardial infarction caused by occlusion of the proximal part of the right coronary artery more often causes LV prolonged relaxation filling pattern; LV filling profile after primary PCI depends on the myocardial necrosis size; LV filling profile assessed one week and 3 months after primary PCI correlates to the CRP levels; severity of diastolic dysfuncti ; on within first 24 hours after PCI is related to the effectiveness of mechanical reperfusion on primary PCI; left ventricle ejection fraction on first day after PCI has important influence on LV filling pattern evaluated 3 months after primary PCI.
