Tytuł: The role of FcεRII (CD23) m regulation of lgE level following surgical trauma
Abstrakt:
IgE plays an important role in the pathogcnesis of alJcrgic diseases. Distinct, transient rise in serum IgE has been also reported following different kinds of trauma. The large body of evidcnce suggests that sFcεRII, known as low-affinity receptor for IgE, shed from the cell membranes, may play a pivotal role in regulation of IgE level. Objective: The aim of our study was to determinc whether transient rise in serum level of IgE following surgical trauma was accompanied by changes in serum sFcεRII. In order to evaluate the source of sCD23 we investigated the expression of CD23 on B cells. Methods: In prospectivc study 33 patients with symptomatic CHD were operated on by CABG with cardiopulmonary bypass. Blood sampling was done beforc surgery and 24, 48, 72, 120 after surgery. FcεRII on B cells and sCD23 were measured by flow cytomctry and enzyme-linkcd immunosorbent assay techniques. Results: We found a significant increasc in total number of Ieucocytes and significant decrease in total number of lymphocytes with increasc in proportion of lymphocytes B (p< 0.001). At the same time the pcrcentage of CD23-positive B cells- (CD19/23+) decreascd significantly (p< 0.001). The exprcssion of CD23 on B cells droppcd significantly (p<0,001) 24 hours after surgery and remained depressed for 72 hours. Serum level of solublc CD23 increased significantly (p<0.05) 24 hours and remained cle ; vated until the end of the observation. All these changes in the immunological status proceeded an increase in serum IgE level (p < 0.001) reaching a peak by the fifth day after surgery (120h).
