Our aim was: a/ To estimate the prognostic value of blood counts of endothelial progenitor cells (EPC) in a homogenous group of medically treated stable angina after a complex elective percutaneous coronary intervention; b/ To assess whether the hypothetical relationship between EPC and prognosis is independent of traditional and nontraditional risk factors. The results suggest that: a/ Circulating EPC deficiency and elevated asymmetric dimethylarginine (ADMA), an endogenous nitric oxide (NO) formation inhibitor, synergistically increase the coronary events risk in pharmacologically treated stable angina patients after elective coronary stenting. b/ The low EPC–high ADMA synergistic prognostic interaction is independent of the traditional and remaining estimated nontraditional risk factors, including other biochemical indices of endothelial dysfunction and parameters of inflammatory activation. c/ The low EPC–high ADMA synergistic prognostic interaction may result from the joint participation of EPC and NO in the maintenance of coronary endothelial integrity. d/ If confirmed by a larger study, the observed associations could be helpful in pre-procedural risk stratification and might possibly be even accentuated for EPC capturing stents coated with anti-CD34 antibodies.
