Tytuł: Clinical relevance of determining chosen immunological parameters in children with different type oh hypogammaglobulinemia
Abstrakt:
Common variable immunodeficiency (CVID), selective IgA deficiency (SIgAD) and transient hypogammaglobulinemia of infancy (THI) are humoral immunodeficiencies characterized by the presence of peripheral B cells. Prospective observation and follow-up studies on memory B-cell subsets in children with hypogammaglobulinemia of unknown etiology revealed that the persistence of low levels of these cells in some children may suggest the CVID diagnosis, but not THI. Hence, determination of memory B-cell subsets might be useful in differential diagnosis of THI and CVID, especially in children younger than 5 years. In SIgAD children it was revealed some shifts in memory B-cell compartment, similar to those observed in CVID children, suggesting additional evidence of a linkage between these entities. Studies on cytokine receptors expression on T-cell subsets revealed that high level of IL-12R expression in CVID children, but not in THI and control group, might be helpful in differential diagnosis of these diseases. The clinical relevance of mutations found in TNFRSF13B gene in children with SIgAD and THI needs to be assessed, whether those mutations predispose to CVID development in later life. Yet, the results of genetic studies on TNFRSF13B gene suggest that determination of mutations in this gene is of limited use in diagnosis and differentiating different types of hypogammaglobulinemia ; in children.
