Abstract:

The aim of the study was to assess diagnostic methods and results of treatment of stomatognathic system dysfunctions according to the methodology developed and applied at the University Dental Clinic (UDC) in Krakow. The study was carried out on a group of 355 patients who were qualified into two groups. Group I consisted of 254 patients, with symptoms of TMD who responded to a questionnaire and were clinically examined. Subgroup IA was created within group I and consisted of 43 patients in whom an examination was conducted at least 6 months after the initial examination. Group II were carried out on 101 subjects who had visited the UDC seeking dental treatment for reasons unrelated to the TMD. Results. The obtained results show that the most common characteristic symptoms of SS dysfunction in the tested group of patients are acoustic effects and pain in the area of the TMJ as well as limited mandible abduction and sensation of muscular fatigue and stiffness. A statistically significant result (p<0.001) was obtained for the dependence between the result of the subjective assessment of the masticatory organ motor components according to the Helkimo anamnestic questionnaire and the clinical examination for pain symptoms in group I. A similar assessment to verify joint sound symptoms clinically confirmed their presence p<0.001). Conclusions: 1. The following symptoms form the clin ; ical basis for diagnosing stomatognathic system dysfunction: pain, acoustic effects in the area of the TMJ, head pain and impaired mandibular range of motion. 2. Diagnosis of SS functional disorders requires confirmation via specialized tests and taking the psychoemotive state into account in the case of patients above 45-years-of-age. 3. The diagnostic procedure methodology should be supplemented with two additional parameters: a pain measurement scale and a tooth grinding index. 4 The methods of treating SS dysfunction in the UDC can be considered partially effective which means that they partially or fully eliminate muscle, joint and head pain symptoms. However they do not always produce an elimination of the accompanying acoustic symptoms.
