Tytuł: The prognostic factors and results of treatment in patients with acromegaly
Abstrakt:
Acromegaly is a rare, chronic disease caused by a GH — secreting pituitary adenoma. The aim of this study was the evaluation of factors influencing the results of the treatment and the frequency of other diseases in acromegaly. The patients with acromegaly treated at the Endocrinology Clinic in Cracow between 1986 and 2009, were subjected to examination (113 patients: 83 women, 30 males, mean age 46,5±14,9yrs). Macroadenoma of pituitary was the cause of disease in 77% patients, microadenoma in 23 % patients. Applied treatment: surgery in 85 patients, radiotherapy after surgery in 14, radiotherapy only in 5, treated with somatostatin analogs after unsuccessful surgery in 49 and treated only with somatostatin analogs in 23. Two groups, effectively and ineffectively operated, were compared to each other and a significant difference between initial GH level (p<0,05), IGF-1 (p<0,001) and the biggest size of pituitary tumor (p<0,05) was reported. There was a significant difference (p<0,05) in GH level before taking the somatostatin analogs in the group where the disease was under control and in the group where the disease was not under control. The significant fall in the GH level (p<0,005) and IGF-1 (p<0,05) in the first year of therapy was reported. It was found out that the most significant factor influencing the development of other diseases is duration of active acromegaly tha ; t is the time of exposition to elevated GH and IGF-1. The proper control of acromegaly allows to reduce the number of co-existing complications in acromegaly. Early diagnosis has significant meaning in acromegaly.
