Title: The analysis of the results of treatment trochanteric fractures obtained with Gamma nail in elderly patients

Abstract:

The subject of this study are problems concerning surgical treatment of trochanteric fractures of the femur in elderly patients using Gamma nails. We did an analysis both X-ray and functional results taking as a control group patients having been treated for a trochanteric fracture with a bundle of Ender’s nails. The general aim of the study has been a finding answer to the question: Does the surgical treatment of trochanteric fractures of the femur in elderly patients using Gamma nails provide a stable bone union and to what extent it allows to regain a pre-traumatic mobility; that being compared to patients having their fracture united with a bundle of Ender’s nails? The aim of the study had been realized over the years 2003 – 2007, and it is based on my own clinical experience during my work at the Orthopedic and Trauma Ward of St. Lukas Hospital in Tarnów. The study group consisted of 194 patients having their fracture united with a Gamma nail. The control group was formed by 153 patients having had a surgery utilizing Ender’s nails. For the purpose of establishing the type of fracture Evans’s classification modified by Jensen was used. An original scale was utilized to evaluate functional and X-ray results of every patient. The stability of bones unions was clinically verified by analyzing mechanical complications in both study groups. Mortality rate and reasons of death w ; ere additional aspects of the study in the population. The analysis of the obtained results of treatment allows to come to the following conclusions: The stability of unions of femoral bones fractures obtained with Gamma nails in elderly patients was good when it comes to stable and unstable pertrochanteric fractures but in case if subtrochanteric fractures it was significantly lower. Pre-traumatic mobility after 1 year following the fracture was regained by 79.5% of patients with stable pertrochanteric fractures, 66% of patients with unstable pertrochanteric fractures and 36.7% of patients with subtrochanteric fractures operated by using the Gamma method. In the study group, in which Gamma nails were used, pre-traumatic mobility after one year was achieved in 72.3% of patients with normal pre-traumatic mobility and only in 27.7% in patients with limited pre-traumatic mobility. Clinical and X-ray results in the group of patients with subtrochanteric fractures united with Gamma nails were better than those united with Ender’s nails. The interpretation of clinical results between the study groups, treated with Gamma and Ender’s method is difficult. The overall mortality rate in the study group was 10.3%. The mortality rate in particular types of fractures was: 4.3% in stable pertrochanteric fractures, 11.4% in unstable pertrochanteric fractures and 19.6% in subtrochanteric fractu ; res.
