Tytuł: Scintigraphic evaluation of salivary gland function in patients with autoimmune thyroid desease and after radioiodine therapy

Abstrakt:

Some similarities between salivary glands and thyroid cause that autoimmune thyroid disease (ATD) very often co-exist with autoimmune sialadenitis. Also Sjögrena syndrome is frequently accompanied by ATD. It is known that salivary glands actively absorb iodine and since the beginning of the treatment by radioactive iodine its destructive effect on the glands has been observed. The aim of this dissertation was to evaluate the influence of treating hyperthyrosis with low doses of radioactive iodine and the influence of autoimmune thyroid disease on salivary glands function, assessed by semiquatitive scintigraphy. The research was conduced on the group of 100 participants, 80 of which ware the patients treated in the outpatient Department of the Endocrinology Clinic CM UJ in Cracow in 2004-2006 because of ATD or hyperthyosis caused by toxic goitre. The patients were divided into 4 20-person groups according to diagnosis. The first group consist of the patients with Graves disease treated with 131J, the second group were the patients treated with 131J for the toxic goiture, the third group- the patients with Graves disease treated with thyreostatics, and the fourth group were the patients with Hashimoto disease. The fifth was the control group of the healthy volunteers chosen from the clinic staff. All participants were examined and accordingly qualified to one of the group listed ; above.Then they were interviewed by the doctor about xerostomy answering 9 standard questions. Next the semiquatitive scintigraphy of the salivary glands was performed using 99m Tc. In the 20 th minute of the examination the sialogogue was given: a 200mg tablet of vitamin C. The changes in radioactivity in salivary glands were evaluated using excreation rate (ER), witch was defined as ER= maximum activity- minimum activity divided by minimum activity and shown in percentage terms. According to ER the degree of salivary gland damage was established in 4grde scale. Next the condition of oral cavity mucosa of the patients was evaluated in the University Dental Institute in Cracow. During the examination microbiological assessment was performed. The results of the research have proved that salivary glands function becomes impaired both as a result of ATD and radioiodine treatment. Semiqantitive scintigraphy has proved to be useful tool for assessing salivary glands function in those conditions. However self assessment of xerostomy is less reliable in comparison with scintigraphy. The micobiological tests have reviled the more frequent occurrence of oral cavity candidosis among the patients exposed to salivary damage, which may be connected with hyposalivation.
