Title: The impact of different doses and forms of hormone therapy on thrombin generation and fibrinolytic activity in postmenopausal women

Abstract:

1) The assessment of the effects of different forms of hormone therapy (HT) on clot characteristics. 2) The evaluation of the effects of different doses of oral HT on thrombin generation and fibrinolysis. 3) The estimation of the effects of different doses of oral HT on thrombin generation and fibrinolytic activity in postmenopausal women depending on the duration of the treatment. Materials and methods: In the studies we have evaluated the influence of different HT forms on the fibrin clot properties and the effects of different HT doses on thrombin generation and fibrinolysis, which were assessed after 24 weeks and 12 months of treatment. Results: 1) HT led to higher maximal absorbency of fibrin gel. Oral treatment was associated with higher absorbency of plasma clots (16%). 2) After 24 weeks in ultra-low-dose group posttreatment clot lysis time (CLT) tended to be shorter with reduced plasminogen activator inhibitor-1 (PAI-1) antigen. Ultra-low-dose HT led to lower F1+2 and FVIII activity. 3) 12-months standard HT led to a higher concentration of prothrombin 1+2 fragment with lower antithrombin activity. Both standard and ultra-low dose HT reduced PAI-1 activity and PAI-1 antigen. Conclusions: 1) Estrogen plus progestogen therapy leads to the formation of thicker and more branched fibrin fibers, particularly during oral administration. 2) 24-weeks ultra-low-dose ; HT enhance fibrinolysis. 3) Ultra-low-dose HT after 12 months revealed positive effects on hemostasis by intensifying fibrinolysis through a decrease in both PAI-1 activity and antigen levels, and with no impact on thrombin generation.
