Despite the rapid advance of medicine between I and II millenium, the non-oncological pathologies of the larynx remain the significant clinical problem in the XXI century. Phoniatric disorders, which dysphonia is an example, pose the considerable challenge for the clinicians in the new millenium due to the fact that significant amount oj professions reqmre the properly functioning larynx. Moreover, the chronic laryngitis remains the most common pathology among many organie causes of voice disorders. Basic forms of treatment are as follow: voice rest, pharmacotherapy or surgical treatment (in case of organie pathologies of the larynx). Voice Handicap Index (VHI) questionnaire and Maximum Phonation Time (MPT) form the basis of phoniatric diagnostics. But videolaryngostroboscopic examination remains the most important diagnostic tool. The electroglottography is being used in diagnostics of voice pathologies. Its improved method - laryngography, may be used for presenting the videostorboscopy-observed pathologies in form of a voice profile. Moreover, the halotherapy or subterraneotherapy (the methods which are based on microclimate o1 subterranean and gro und salt cham bers) may be used for optimization of treatment of non-oncological laryngeal pathologies. The treatment with mentioned above methods is known to the humanity since the ancient times e.g. classical Greece or the Roman ; Empire. In case of Poland subterranean '' Wieliczka" Salt Mine Health resort is an example of a place where using these methods is possible. In 1964 the first of its kind in the world subterranean allergological sanatorium was established there. The aim of this study was to prove the efficiency of subterraneotherapy in treatment of chronic laryngitis on example of the subterranean "Wieliczka" Salt Mine Health Resort. Total 73 patients with chronic laryngitis were examined and the study consisted of 3 stages: I. Phoniatric Outpatient Clinic of the University Hospital of Cracow- medical history, VHI questionnaire, defining MPT and videostroboscopic examination . II. Institute o Technology of the Pedagogical University of Cracowlaryngographic examination, de:fining voice profile. III. "Wieliczka" Salt Mine Health Resort - subterraneotherapy. All patients underwent conservative treatment (pharmacotherapy) and/or surgical treatment. Additionally 43 patients were referred for treatment in subterranean salt chambers in sanitarium in Wieliczka. They constituted the research group ofthis study, while the other 30 patients have continued the therapy with help of conservative treatment and they constituted the control group of this .study. All patients enlisted in the study reported for control videostroboscopic examination 6 months after the end oj therapy, while 46 out of 73 patients r ; eported for control laryngography. The improvement occurrence frequency in patients treated conservatively (after treatment in "Wieliczka" Salt Mine Health Resort) was present in bigger number of videostroboscopic-analysed parameters ( 4 statistically significant parameters- vibration amplitude, mucosal wave, glottal stop and dry mucous membran es) in comparison to patients after the surgical treatment ( only I statistically signi:ficant parameter - dry mucuous membranes). The study demonstrated that undergoing subterraneotherapy signi:ficantly affects the extending the MPT in comparison to using only basie forms of treatment. The decrease of average total VHI points from 41,99 pt. to 27,38 pt. was observed as well. The conclusions are original due to the limited number of studies regarding the impact of subterraneotherapy on local condition and function of the larynx. Additionally the usage of laryngography enabled to show the full picture of voice changes after different forms of treatment i.e. conservative, surgical and balneological. The study, which is the subject of this doctoral dissertation, is an example how important role the natura} forms of treatment can play in an optimization Jof contemporary methods. U sing subterranean salt chambers is the example of mentioned above methods.
