Abstract:

The aim of the study was to determine the impact of the etiology of mitral defect, right ventricular function and the type of surgical approach on the occurrence of periprocedural death and perioperative complications. Two populations of patients were assessed 1.Patients with severe organic and functional mitral regurgitation qualified for cardiac surgery, 2.Patients with severe aortic valve stenosis and coexisting coronary artery disease requiring interventional or surgical treatment, including patients with ACS. Results: 1.The etiology of mitral defect was not demonstrated as a risk factor for perioperative complications and 30-day mortality. 2. It was shown that the TAPSE/RVSP index value below 0.42 mm/mmHg was a predictor of death after correction for age and sex. 3.The hybrid procedure showed similar morbidity and mortality and may be an alternative to conventional AVR and CABG through full sternotomy in selected patients. Conclusions: Risk factors related to the patients that should be taken into account, are comorbidities, echocardiographic parameters of right ventricular dysfunction, and factors related to the cardiac surgery itself, including the type of surgical approach, valve repair vs. replacement.
