Title: Pulmonary valve morphology in light of modern catheter ablation methodology

Abstract:

Over the past decades, creating a thorough assessment of the pulmonary valve’s (PV’s) geometry and morphology received little interest compared to similar efforts in the aortic valve. However, the PV has been gaining increased attention from clinicians (cardiac surgeons and invasive cardiologists), since new treatment methods for ventricular arrhythmias from the right ventricular outflow tract (RVOT) have been developed lately. Thus, the purpose of this doctoral dissertation was to provide a comprehensive macro and microscopic description of PV anatomy and morphometric variability in the context of valvular and supravalvular interventions. To achieve this goal, 182 pulmonary trunks obtained from autopsied human hearts were investigated macroscopically, while additional 65 pulmonary trunks obtained in the same manner were investigated microscopically. This study analyses the morphometric and morphologic variability of the PV on three distinct sectional levels, consequently introducing novel dimensions which weren’t previously described in the literature. These parameters constitute important context for clinical studies as well as engineering projects aimed to create synthetic valve models. Furthermore this study focused on microscopic analysis of myocardial sleeves extending into the sinus walls of the PV. These structures are suspected to be the anatomical origin of idiopath ; ic ventricular arrhythmias, thus being targeted in ablation procedures. This study verifies that myocardial extensions are in fact common anatomical entities. Moreover, it provides detailed morphometric and topographic characteristic of myocardial sleeves. Results of this dissertation will enhance our understating of RVOT origin arrhythmias, contributing to safety and efficacy of catheter ablation procedures performed within this region.
