Tytuł: Comparison of the effects of two approach techniques for hip joint replacement on functional status and selected posturatmeasures in patients with coxarthrosis

Abstrakt:

hip endoprosthesis is a surgical procedure undertaken to relieve pain and restore function to a joint affected by osteoarthritis. The number of such procedures 1s steadily nsrng, with patient expectations for outcomes likewise becoming more demanding. Additional!;', these procedures are increasingly indicated m relatively younger population. Therefore, there 1s a growing interest in exploring mininially invasive techniques. The purpose ofthis siudy is to compare the value of minimally invasive anterior approach (DAA) with direct lateral approach (DLA). Sixty-nine individuals were enrolled in this comparative study. Patients were divided into 2 groups: a DLA group of 36 patients operated using direct lateral approach, and a DAA group of 33 patients operated usmg minima\ly mvas1ve anterior approach. The following postoperative parameters were compared: pain according to the NRS scale, markers of muscle tissue damage ( creatine kinase and myoglobin), inflanmmtory parameters (CRP), perioperative blood loss, TUG and Chair functional tests, WOMAC and HHS scales, acetabular component anteversion and inclination, and postoperative limb length discrepance (LLD). No significant reduction in pain was observed in the DAA patient group. Significantly higher levels of muscle tissue damage markers, inflanJIBatory parameters, and higher perioperative blood loss were obser ; ved. Significantly better TUG functional test scores were shown, but there was no significant difference in the Chair Test. No significantly better results were observed for the HHS and WOMAC scales. Significantly higher values of anteversion and significantly !ower values of acetabular component inclination were observed. Significantly !ower values of !ower limb length discrepance are observed. The use of minirnally invasive anterior approach shows better results in only some of the assessed parameters. DAA access is a safe technique and may provide some advantage in short-term outcomes. However, it is a technically demanding procedure with a long learning curve. Endoprosthesis perfonned using anterior approach needs further improvement, and more complex cases should be performed usmg a standard technique that provides predictable results and 1s optimally mastered by the operator.
