This PhD thesis examines the key reforms of coordinated care in Poland and compares them with the Kaiser Permanente model, which - by differentiating between levels of healthcare needs - offers a simple framework for comparing reform initiatives and identifying gaps in meeting those needs. The analysis shows that initiatives undertaken so far have focused on specialized care, largely neglecting primary healthcare (PHC), despite its crucial role in care coordination in Western Europe and the USA. An exception was the PHC PLUS pilot (2018-2021), which focused on strengthening PHC and seemingly encompassed all levels of the Kaiser Permanente model, meeting healthcare needs of varying levels of complexity, as well as placing strong emphasis on population-based disease prevention and health promotion. Nevertheless, existing coordinated care programmes remain relevant, and the current reform of PHC should be considered in conjunction with these programmes and with other reform proposals, such as the 75+ Health Centres and County Health Centres models, which go beyond PHC and beyond the healthcare sector. Due to significant and persistent health disparities between regions, adapting service delivery to the needs of local populations, as suggested by the County Health Centres model, may lead to actual improvement in health outcomes in Poland.
