Abstract:
Chronic illnesses constitute one of the most common problems of medicine, which developing countries are facing. A chronic illness affects all functional dimensions of a person. The present study attempts to identify the factors determining the readiness of patients with a chronic somatic illness to be discharged from hospital. The study was conducted on 209 patients of both sexes, aged 18 to 93, treated at the John Paul II Podhale Region Specialty Hospital in Nowy Targ in 3 medical treatment wards. 97 patients from the Rheumatology Ward of the Internal Medicine Department, 100 patients from the Stroke Treatment Ward of the Department of Neurology and 12 patients from the Department of Cardiology. The study used: Proprietary questionnaire, Questionnaire from medical records, Standardized tools: A) Readiness for Discharge Questionnaire, B) Nottingham Health Profile (NHP) C) Acceptance of Illness Scale (AIS), D) Multidimensional Health Locus of Controlform B (MHLC-B). Results with p<0,05 were considered statistically relevant. The analysis of hospital discharge readiness, both in its respective categories and the overall evaluation, conducted by the patient and the researcher, showed in each case that the patient evaluated his or her discharge readiness higher than the researcher. The overall assessment of readiness for discharge from the hospital conducted b ; y the patient with a chronic disease as well as the examiner was at a satisfactory level, with the patient’s subjective assessment being higher than the results obtained in the objective assessment.
