Title: Prognostic factors and outcome assessment in patients with myocardial infarction


The aim of the doctoral thesis was to search for new predictors for the assessment of prognosis in patients with myocardial infarction. The doctoral thesis is a series of 3 original publications and a narrative review. The work consists of a prospective part and a retrospective part. The narrative review presented the current state of knowledge on circulating biomarkers of post-infarction left ventricular remodeling. In the prospective part, the level of selected biomarkers in the acute phase of myocardial infarction (MR-proADM, Notch-1, syndecan-4, myeloperoxidase, s100 protein, ST2, interleukin-6, lactate dehydrogenase, C-reactive protein) was examined. The relationship between the level of biomarkers and the remodeling of the left ventricle during the 6-month follow-up, assessed by echocardiography with the use of 3D imaging, was analyzed. The above analysis shows that LDH and MR-proADM are promising markers. In the retrospective part, the role of the CHA2DS2-VASc scale in the assessment of prognosis in patients with myocardial infarction was examined. The study showed the predictive value of the CHA2DS2- VASc and R2-CHA2DS2-VASc scales for all-cause mortality and recurrent myocardial infarction. Another issue in this part of the study was the analysis of the length of hospitalization and factors influencing the length of stay in hospital in patients with ; myocardial infarction. Patient age, left ventricular ejection fraction, and the presence of ST-segment elevation myocardial infarction and multivessel coronary disease were independent predictors of hospitalization for ≥ 8 days.
