Tytuł: Selected problems related to increasing incidence of type 1 diabetes mellitus in children and adolescents
Abstrakt:
Diabetes mellitus type 1 (DM1) is one of the most common chronic diseases in the world population of children and adolescents. Each year, it is diagnosed in approximately 96,000 children under the age of 15 worldwide. So far, no DM1 incidence registry has been established in Poland, therefore, there are insufficient data to determine the national incidence. In own research, which analyzed the medical records of 636 patients aged 0-14 years from Małopolska in 1987-2012, it was shown that the incidence of DM1 increased from 5.2 to 21.9 / 100,000 over 26 years of observation. from the hypotheses trying to explain the increased incidence of DM1 among the youngest children is the TJ Wilkin accelerator hypothesis, according to which the greater body weight of children could predispose to the early manifestation of insulin deficiency. In the own study verifying this hypothesis, an upward trend in the incidence of DM1 was shown, but without a statistically significant decrease in the age of diagnosis and the influence of excess body weight measured with the BMI-SDS index on the age of diabetes diagnosis. Another problem associated with the increased incidence of DM1 is the constant relatively high proportion of severely hospitalized patients with advanced DKA. In the last questionnaire survey, the theoretical and practical knowledge of doctors working in Primary Healthcare was assessed ; , and the result of this analysis showed the need for continuous education and increased vigilance of doctors in recognizing early DM1 symptoms in children.
