Tytuł: Frequency and types of arrhythmia in pregnant women with structural heart diseases, cardio-vascular outcomes during pregnancy and postpartum period
Abstrakt:
Cardiovascular diseases constitute the main cause of pregnant mortality in developed countries. Presence of structural heart disease and later age of women at first pregnancy increases the risk of cardiac outcomes and arrhythmias are among the most common complications. The aim of the study was to assess the frequency, types and kinetic profile of cardiac arrhythmia, based on 24h Holter-ECG monitoring during each trimester and postpartum period, in pregnant women with cardiovascular diseases and different cardiac risk according to mWHO classification. The frequency of cardiovascular and obstetric outcomes in the study population was also analyzed. The incidence of arrhythmias was 45.6%. Ventricular arrhythmia occured in 1/3 (31,1%) population: episodes of nsVT in 17 (16,5%), sVT in 2 (1,94%), PVCs>720/24h in 22 (22,4%) women. Supraventricular arrhythmia occured in 1/4 (24,3%): episodes of SVT in 12 (11,7%), SVE > 720/24h in 18 (17,5%), AF in 1 (0,97%), AT in 2 (1,94%) women. The kinetic profile of arrhythmia differed between women with lower and higher cardiac risk. The frequency of ventricular arrhythmia increased during pregnancy regardless of the mWHO class, whereas supraventricular arrhythmia behaved differently: it increased in higher cardiac risk group (mWHO class II-III, III, IV) and decreased in pregnant women with lower cardiac risk (mWHO class I, II). The incidence o ; f cardiac outcomes in study population was 4.9%, and initial value of LVEF (systemic) was an independent predictor. The incidence of obstetric outcomes was 24.3%, the independent predictors were aortic valve and aortic root pathology and pre-existing hypertension.
