Abstrakt:

Acute kidney injury (AKI) is a frequent complication of severe acute pancreatitis (AP) which significantly increases mortality. The aim of the study was to assess the selected novel biomarkers of renal injury in patients with early stage of AP regarding the diagnostic utility for AKI complicating AP. The study concentrated on two markers that had not been studied in AP previously: beta-trace protein (BTP) in serum and kidney injury molecule-1 (KIM-1) in urine. 73 patients admitted to hospital within first 24 hours from the onset of AP symptoms were recruited in two medical centers. Laboratory tests were performed in samples collected on first, second and third day of hospital stay. AKI was diagnosed in 13 patients. Serum BTP concentrations during the study were well correlated with serum urea, creatinine, cystatin C and uromodulin, and correlated with serum NGAL on days 1 and 3 of the study but did not correlate with urinary NGAL. The patients who developed AKI had higher serum BTP. Urinary KIM-1 concentrations were not correlated with serum creatinine and were not higher in patients with AKI, however, urinary KIM-1 was associated with the severity of inflammation in the early phase of AP. The obtained results support the usefulness of serum BTP, cystatin C and both serum and urinary NGAL measurements for the prognosis of AKI complicating the early phase of AP.
