Tytuł: Biochemical assessment of postoperative pain modulators in children undergoing extensive spinal surgeries
Abstrakt:
The main goal of the project was to comprehensively assess alteration in serum concentration of biochemical pain modulators like proinflammatory cytokines, endogenous opioids and endocannabinoids depending on the type of anesthesia protocol, with particular emphasis on intravenous (iv) lidocaine administered as a co-analgesic, in children undergoing extensive spinal surgeries. An additional aim of this project was to analyze predictors that may affect the postoperative comfort of children. It was observed that proinflammatory molecules, endoopioids and endocannabinoids play a significant role in patomechanism of postoperative pain and their concentration is modulated by perioperatively iv administrated lidocaine. Additionally, the lower demand for morphine during postoperative period in children who received iv lidocaine was reported. Base on the above-described observations, it was concluded that perioperatively, iv administrated lidocaine seems to be a promising option for multimodal postoperative pain control. Moreover, the alterations in the routinely measured parameters such as the concentration of hemoglobin, hematocrit and mean blood pressure during surgery which might be the predictors of the intensity of postoperative pain were described. They could be useful during cohorting pediatric patients to risk groups with increased postoperative pain intensity and thus could ; be helpful in the rapid treatment implementation.
