Tytuł: Clinical anatomy of human heart venous system
Abstrakt:
Following rapid development of intervention procedures for treatment of arrhythmias and heart failure, the demand for accurate knowledge of human venous morphology has increased. This knowledge is particularly important for operators performing electrophysiological procedures or implantation of cardiac resynchronization therapy systems. The purpose of this doctoral dissertation was to provide: 1) morphometric characteristics of coronary sinus tributaries, with particular emphasis on angular vein inflow; 2) topographic description of the left atrial oblique vein (Marshall vein), including the relationship between the Marshall vein and the left-sided pulmonary vein and left atrial appendage; 3) anatomical characteristics of the structure and location of the Vieussens valve; and 4) contrast enhanced computed tomography assessment for in vivo diagnosis of Vieussens valve presence. To achieve these goals, 200 autopsy heart preparations and 114 contrast enhanced tomography examinations were analyzed. The study presents a comprehensive morphometric analysis of coronary sinus tributaries, including anatomical nomenclature systematization and providing clinical guidance for electrocardiological procedures. Due diligence has been given to the topography of the left atrial oblique vein, distinguishing four types of its course. The contrast enhanced computed tomography performed according ; to the standard protocol has proved to be useful tool in imaging large Vieussens valves, that can be significant obstacle to cardiac venous catheterization procedures.
