Title: Nonalcoholic fatty liver disease as the risk factor of macroangiopathy complications in patients with type 2 diabetes

Abstract:

Nonalcoholic fatty liver disease (NAFLD) is a growing clinical problem connected with diabetes type 2 and visceral obesity, that can lead to cardiovascular complications in DM2 patients. The prevalence of NAFLD in our clinical observation was around 60-85%. The patients with NAFLD compared to those without it, were characterized by bigger waist circumference, higher BMI and, as we expected, higher level of transaminases and GGTP. In the lipid profile, we observed lower HDLcholesterol in NAFLD group. Partial regression analysis, revealed that NAFLD in patients with diabetes type 2 was associated with central obesity and dyslipidemia. However, we did not find any association between NAFLD and the level of glycemic controlled. In our group of DM2 patients, those with DM2 and NAFLD were characterized by a higher requirement for cardiovascular revascularization procedures compared to patients without fatty liver. We also examined clinical and biochemical factors associated with inflammatory plasma protein pentraxin 3 (PTX3) concentrations. PTX3 is also a marker of cardiovascular risk in DM2 patients. In the whole group of DM2 patients PTX3 level was associated with total cholesterol, low density lipoprotein cholesterol (LDL - C), apolipoprotein (APO) B100, APO C3, triglyceride concentrations, and waist circumference after adjustment for age and sex. Partial regression analysis re ; vealed, that in the NAFLD subgroup, serum pentraxin 3 correlated positively and significantly with total and LDL-cholesterol, triglycerides, apolipoprotein C3 and apolipoprotein B100. Associations of PTX3 with apolipoproteins were observed only in the NAFLD group. It has been shown that pentraxin 3 as a marker of cardiovascular complications correlates significantly with the level of apolipoproteins that are contributed to atherogenic actions.
