About 5% of adult patients requiring dental treatment are on antithrombotic medication, most frequently aspirin Discontinuation of the ASA therapy prior to extraction of a tooth is a common practice The aim of the study was to determine: a. whether the patient subjected to extraction of a tooth without ASA therapy cessation is sufficiently secured against postextraction bleeding by application of local measures enhancing hemostasis b. the effectivness of three methods of local hemostasis enhancement in preventing the bleeding complications after extraction of a tooth (suturing, suturing and gelatin sponge application, suturing and mouth rinsing with tranexamic acid) in patients on ASA therapy This prospective clinical study was conducted in the Department of Oral Surgery at the Medical College of the Jagiellonian University in Krakow and included 120 patients (48 males and 72 females, aged 47-70). Conclusions 1. Aspirin therapy cessation prior to a tooth extraction is futile 2. In comparison with suturing alone, mouth rinsing with TXA and gelatin sponge used with wound suturing provide better postprocedural hemostasis 3. With regard to correlation between severity of bleeding during extraction, bleeding time and the presence of inflammation of periodontal tissue may be observed, full mouth disinfection prior to extraction of a tooth should be performed, especially in patients with hemostatic disorders
