Title: Role of troponin and the N-terminal pro B-type natriuretic peptide in the pathophysiology of hypertrophic cardiomyopathy
Abstract:
Hypertrophic cardiomyopathy (HCM) is a primary disease of the heart muscle that leads to segmental or diffuse thickening of the left ventricular walls. Recent studies in a group of patients with HCM found that about 50% of patients have elevated levels of the biomarker of myocardial ischemia - cardiac troponin. The level of N-terminal pro B-type natriuretic peptide (NT-proBNP) is significantly increased in most patients with hypertrophic cardiomyopathy. The aim of the study was to search for previously unknown connections between biomarker levels and hemodynamic disorders in patients with HCM. It was prospective, single-center study. Among enrolled patients, two outpatient visits were carried out, a transthoracic echocardiography was performed, and blood samples were collected to determine the levels of creatinine, NT-proBNP and high-sensitivity Troponin I (hs-TnI). Based on the conducted tests, the reliability of hs-TnI level tests in monitoring symptomatic and asymptomatic patients was indicated, moreover a higher diagnostic value of the standardized NT-proBNP (NT-proBNP / ULN) level tests was pointed out, compared to the absolute value of NT-proBNP in monitoring patients with HCM with both, angina and with silent ischemia. Elevated NT-proBNP / ULN in patients with HCM has been shown to predict the presence of left atrial enlargement and increased likelihood of pulmonary hype ; rtension in echocardiography.
