Tytuł: Urinary tract infections in the material of regional hospital and their impact on the kidney function and patients' health prognosis

Abstrakt:

The doctoral thesis was realized through the publication of 3 articles regarding the subject of urinary tract infections (UTI). The aim of the study was a retrospective analysis of UTI in the material of internal-nephrological department of the regional hospital in 2013-2015. The analysis covered 498 medical records of patients aged 18-101 with the diagnosis of UTI. In the 1st article, the occurrence and course of acute kidney injury (AKI) were assessed, taking into account risk factors for AKI development and prognosis. The relationship between baseline eGFR and the risk of AKI was also analysed, as well as the impact of AKI degree on the occurrence of death. In the 2nd article, we determined the usefulness of C-reactive protein and procalcitonin in an early diagnosis of UTI and urosepsis. Moreover, the risk of death depending on the concentration of these markers at admission were assessed, as well as their sensitivity, specificity and predictive value. In the last article, we analysed the etiology and risk factors of UTI, as well as drug susceptibility of E. coli as the most frequent pathogen in various age groups.UTI are one of the most common infections in the human population, while AKI is an important clinical problem. Multiple factors responsible for the development of these infections and increasing antibiotic resistance justify the need for monitoring of UTI within va ; rious health care units. The presented study may be useful in order to identify and eliminate the risk factors of UTI. In addition, this material may be helpful in creating a hospital formulary for empirical treatment of UTI, contributing to more effective treatment, reducing the number of complications and improving the health prognosis of patients.
