Title: The assessment of the usefulness of prenatal magnetic resonance imaging in the diagnosis of central nervous system defects

Abstract:

Central nervous system (CNS) abnormalities are cause about 40% of infant deaths in the first year of life. In case of detection of abnormalities by ultrasound a pregnant woman should be offered prenatal magnetic resonance imaging (pMRI). The aims of the study were: to evaluate the effectiveness of pMRI in the diagnosis of selected fetal CNS defects. To evaluate the relationship between the timing of the performed tests and the accuracy of diagnoses. To assess the possibility of replacing postnatal tests with prenatal magnetic resonance. The prospective and observational study was conducted between 2014-2017 at the University Hospital Obstetrics and Perinatology Clinic in Krakow. Patients with suspected CNS defects of the fetus were qualified for pMRI in the third trimester of pregnancy. 60 patients were included in the study group. On the basis of the analysis, the following conclusions were made:1. pMRI is characterized by:• high accuracy in the diagnosis of ventriculomegaly,• fairly high accuracy in the diagnosis of midline brain malformation, abnormalities of the posterior cranial fossa, disorders of gyrification, neuronal migration and neuronal proliferation.2. Prenatal MRI is characterized by high accuracy in the evaluation CNS structurein case of non-CNS defects, at the same time pMRI is characterized by low accuracy in the diagnosis of complex brain defects. It was found ; that the number of inaccurate results increased with the prolongation of the interval between pre- and postnatal tests.3. There was no correlation between the week of pregnancy of pMRI and the accuracy of the diagnosis. However, it should be noted that the group was homogeneous and pMRI was conducted at similar time periods.4. No evidence was found suggesting the replacement of postnatal tests with pMRI.
