Title: Effects of early rehabilitation on outcomes after arthroscopic single and double bundle ACL reconstruction

Abstract:

Because of functional meaning of knee joint together with increasing number of injuries constant searching of optimal treatment methods, both surgical and postoperative rehabilitation, is necessary. Goal of postoperative rehabilitation is to restore normal function of knee joint. Good treatment programme should contain mostly many different elements of kinezytherapy including proprioception training. Goal of this study is to compare treatment results in patients after single and double bundle ACL reconstruction treated with the same rehabilitation programme according to author of this study. Data analysis included 94 patients qualified to the study who fulfilled criteria and finished the whole treatment programme. The first group consisted of 49 patients after single bundle ACL reconstruction (average age 36,5), 17 women and 32 men. The second group average age 35,6) consisted of 45 patients after double bundle ACL reconstruction (17 women and 28 men). Evaluation was done before operation (initial test), 7 days after the operation (test 1), 14 days after (test 2), 21 days (test 3) and 4 weeks after the operation (test 4), 6 weeks after (test 5) and 12 weeks after (test 6). Measured items were: muscle strength and ROM of knee joint, arthrometry, balance, pain and subjective evaluation with Lysholm scale and 2000 IKDC Form. Ballotable patella sign was also checked and information ; about puncture of knee joint was noted. ; In group 1 results of passive and active knee joint flexion were statistically significantly better after 4 and 6 weeks after the operation. In all treated patients after 12 weeks normal strength of knee extensors was achieved. There was statistically significant difference of knee flexors after 12 weeks- patients form group 1 had better results. Double bundle ACL reconstruction (group 2) resulted in statistically significantly better stability of knee joint in arthrometric evaluation after treating patients with the same rehabilitation programme. No statistically significant differences were found in static and dynamic balance between the two groups. In patients form group 1 pain level was significantly less in all the tests. Results of subjective evaluation of knee joint functioning according to modified Lysholm scale and 2000 IKDC Form were comparable and shown higher agility of patients form group 1 which was statistically significant. Results of ballotment were significantly better in patients from group 1 in tests after 3 and 4 weeks.The study shown that patients after double bundle ACL reconstruction (group 2) achieved worse results in majority of the measured items despite of better stability of knee joint. There should be more concern about decreasing of oedema and pain which probably could help to achieve better results in th ; is group.
