Equivocal non-pigmented (pink) skin lesions present difficult management for physicians in skin cancer screening settings. Combined application of digital dermoscopy and reflectance confocal microscopy (RCM) can improve diagnostic sensitivity and specificity in this subset of lesions. Integration of a store-andforward (SAF) telemedicine platform to exchange tele-dermoscopy and tele-confocal images can connect doctors at a distance for expert consultation and diagnosis. The aim of the first study was to improve diagnostic sensitivity particularly for the detection of featureless incipient amelanotic melanomas by comparing single reader versus double reader application. Automated referral of lesions for removal when two readers had discordant management decisions resulted in an overall sensitivity of 98.3% and specificity of 42.7%. In the second study under similar conditions, a high percentage of basal cell carcinomas were correctly diagnosed with a sensitivity of 77.2% and a specificity of 96.6% and PPV of 94.6%.
