Title: Diagnostic and prognostic significance of speckle tracking echocardiography in patients with aortic stenosis and preserved left ventricle ejection fraction

Abstract:

Left ventricle (LV) function assessment is important for qualification to aortic valve replacement (AVR). Two-dimensional speckle tracking echocardiography (2D-STE) enables global end regional LV systolic function measurement. The aim was to describe clinical and echocardiographic characteristics of patients with aortic stenosis (AS) and preserved EF, changes in LV systolic function after AVR and prognostic significance of the analyzed parameters. Eighty-five AS patients were included. Longitudinal strain (LS) in all 17 LV segments and global longitudinal peak strain (GLPS) were measured with the use of 2D-STE. Mitral annular plane systolic excursion (MAPSE) and peak annular systolic velocity (S’) was measured using conventional techniques. Death and non-death major adverse cardiovascular events (MACE) were analyzed. A year after AVR control echocardiography was performed. In AS patients GLPS, S’ and MAPSE were decreased. GLPS, S’ and MAPS negatively correlated with AS severity, were lower in symptomatic AS patients in comparison to asymptomatic ones and improved after AVR. Parameters obtained with the use 2D-STE were not predictive of death and composite endpoint. Feasibility of 2D-STE was high both before and after AVR.
