Tytuł: The clinical value of a PET-CT examination in the treatment of recurrent ovarian cancer

Abstrakt:

Patients who were originally treated for ovarian cancer, and who achieved a complete remission, are subject to follow-up observation with a view to a detection of the disease recurrence. The time in which the recurrence may occur chiefly depends on the degree of clinical advancement, as well as on the radicalness of the primary surgery. Apparently, the main factor increasing the effectiveness of the recurrent cancer treatment is an early detection.For the trial to be conducted, the following objectives were set: checking the role of a PET-CT examination in monitoring patients after a completed combined treatment of advanced ovarian cancer; a calculation of the SUV value, which would – with high probability – point to an active process of carcinogenesis; and an establishment of the usefulness of a PET-CT examination in the process of planning the extent of a surgery in patients with a diagnosed recurrence after a primary cytoreductive surgery involving multiple organs. The trial comprised 140 patients treated in the years 2010-2014 according to the standard protocol for the treatment of ovarian cancer, which embraced a primary multi-organ cytoreduction as the first treatment stage and an adjuvant chemotherapy.The treatment group was made up of 85/140 (61%) patients monitored according to a specified diagnostic standard, which was supplemented with a PET-CT examination carried ou ; t at an interval of at least one year.The control group made up of 55/140 (39%) patients - who were monitored according to a standard protocol - were subject to an imaging examination different than a PET-CT.On the basis of the obtained results it was determined that the more accurate detection of neoplastic lesions enables a more precise and targeted cytoreducion, thanks to which the survival rate was higher in the treatment group than in the control group.The PET-CT examination made it possible to pinpoint the spot of recurrence more precisely, which enabled an earlier process of planning the extent and technique of a surgery.
