The analysis of the results of pharmacological treatment of infantil haemangiomas

Abstract:

Infantile hemangiomas are the most common, soft-tissue tumors of childhood. The aim of this study was to assess: a) the efficacy of propranolol and corticosteroids in the treatment of infantile hemangiomas by determining the proportion of early response to the applied therapy and the ratio of disappearance of infantile hemangiomas after the pharmacological treatment; b) the relationship between the degree of early response to therapy or the degree of disappearance of infantile hemangiomas after the pharmacological treatment and: the age of the children at baseline, the location and the type of the hemangiomas, the child’s sex, the birth weight, the duration of pregnancy, the total duration of the treatment, the number of points obtained in APGAR scale, the number of successive pregnancies and the type of delivery. The additional objective was to compare the incidence of complications during the pharmacological treatment of children with infantile hemangiomas. The subject of the analysis were medical and photographic records of 97 children with infantile hemangiomas. 24 of the patients were treated with corticosteroids and 73 children were treated with propranolol. A significant relationship between the type of the pharmacological therapy and the therapeutic effect was identified one month after the beginning of the treatment and at the end of treatment. The application of propr ; anolol in the treatment of infantile hemangiomas results in a higher proportion of children with early positive response than the use of corticosteroids. Also, the propranolol therapy makes it possible to achieve a greater degree of hemangioma involution at the end of the treatment. In addition, infantile hemangiomas located in the region of the head treated with propranolol show a greater degree of involution compared to tumors located in other areas of the body. Regardless of the drug being used, there was no relationship between early response to treatment or the degree of disappearance of infantile hemangiomas after the pharmacological therapy and the type of the hemangioma, the age of the child at the beginning of treatment, the child's sex and the birth weight, the duration of pregnancy, the number of points in APGAR scale, the number of successive pregnancies and the type of delivery. In 54,2 % children treated with corticosteroids, the following complications occurred: moon face, hypertension, fungal urinary tract infection, infection of ulcerated hemangiomas, oral trush (two patients). Only in 9,6 % of the children treated with propranolol, the following complications were observed: asymptomatic, morning hypoglycemia, and gastroesophageal reflux.
