Abstract:

In the last two decades many studies indicated the specificity of a type of schizophrenia which incorporates primary, persistent negative symptoms. The aim of this study is the appraisal of the prevalence and stability of negative symptoms (NS) identified with four different methods, as well as the assessment of their relationships with treatment outcomes after 3, 7, 12 and 20 years. 50 patients hospitalized for the first time with schizophrenia DSM III diagnosis participated in the study. The negative symptoms were selected based on BPRS assessment, where subsequent methods were gradually more exclusive of secondary sources of their origin. The prevalence of primary persistent NS after the first episode of the illness equaled 6%. The prevalence of secondary NS after 20 years reached 38%, and primary NS 12%. Time stability of NS identified with four different methods after a year since the discharge from the first hospitalization decreased the more selective were the criteria excluding secondary NS. In the same manner, time stability and the strength of associations with treatment outcomes also decreased. The obtained results indicate, that primary, persistent NS identified at the time of the first schizophrenia episode are not stable over time and have no prognostic significance.
