Title: The influence of chronic total occlusion revascularization on left ventricular function and clinical presentation in patients with coronary artery disease

Abstract:

BACKGROUND: Chronic total occlusion (CTO) is defined as complete obstruction of native coronary artery with duration longer than 3 months. There is a lack of clear consensus concerning optimal management strategy and methods of therapy. OBJECTIVES: The aim of the study was to compare the effects of CTO treatment in patients after successful PCI CTO, and in patients pharmacological treatment. METHODS: 60 patients with stable coronary artery disease and angiographically diagnosed CTO were included to prospective registry and assigned to two groups. Group I - patients after successful PCI CTO, group II - patients with pharmacological treatment and after an unsuccessful PCI CTO. Follow-up at 1 and 6 month included in all patients: severity of angina (CCS class), angina-related quality of life (visual analogue scale), cardiac function (echocardiography), and exercise tolerance and ischemia evaluation (ECG exercise test).RESULTS: In group I during the 6 month follow-up statistically significant increase in LVEF was observed. After 6 months 100% of patients in group I were in CCS class I. In group I the ECG exercise test performed after 6 month showed statistically significant clinical improvement (100% were clinically negative), in group II prolongation of time to onset of pain in the chest was found. In the group I there is significant improvement of the quality of life . CONCLUSION ; S: Successful CTO recanalization leads to significant improvement LVEF. Patients after successful PCI of CTO suffer less from symptoms of coronary artery disease, and have also better quality of life than patients with pharmacological treatment exclusively.
